Mishap Investigation and Corrections Process Review

Even in the most seemingly straightforward accidents, seldom is there only a single cause. The process of conducting investigations and examining causal factors must be done with rigor. Join the Safety and Health Learning Alliance as speakers topics such as

  • Who has investigation authority?
  • How deep does an organization go in determining causal factors?
  • How does an organization prescribe corrective actions or recommendations?

Our panel members include

  • Johnnie Banks — Chemical Incident Investigator, U.S. Chemical Safety and Hazard Investigation Board
  • Hugh O'Connor — Special Agent, Bureau of Alcohol, Tobacco, Firearms and Explosives
  • John Vorderbrueggen — Division Chief, Pipeline and Hazardous Materials Investigations, U.S. National Transportation Safety Board
